📋 Admission Requirements

  For Preschool and Grade 1 Applicants:

• Nursery: Must be 4 years old by June 1
• Kindergarten: Must be 5 years old by June 1
• Grade 1: Must be 6 years old by June 1
• Submit the latest Form 138 (Report Card)

For Elementary Transferees:

Please prepare the following documents:

• Original or certified true copy of Birth Certificate
• Form 138 (Report Card)
• Form 137 (Learner’s Permanent Record)
• LRN Certificate (Learner Reference Number)
• Completed Registration and Enrollment Forms
• Required school supplies (folders, envelopes, etc.)

For Senior High School Transferees:

In addition to the documents above, please also submit:

• ESC Certificate (if applicable)
• Tax Exemption Declaration
• Use the correct folder color:
  • Grade 11: Blue folder
  • Grade 12: Pink folder

🆕 For New Students and Transferees (All Levels)

  Follow these steps for a successful enrollment:

1. Visit the Guidance Office to receive your enrollment checklist
2. Submit required documents to the Registrar’s Office
3. Take the entrance assessment and attend an interview
4. Complete and submit the Registration Form
5. Pay the assessed enrollment fees
6. Present the official receipt to the Registrar
7. Distribute copies of the enrollment form and submit one to your class Adviser on the first day of school
